Referrals

You will need a current referral from your physician or medical provider (primary care doctor, orthopedic surgeon, neurologist, cardiologist, nurse practitioner, physician assistant, podiatrist chiropractor, etc.) for your insurance company to pay for your physical therapy treatments.

BeWell Home Physical Therapy will accept an initial referral by fax, phone or by email by your medical provider and we will obtain the signed prescription. You may also bring in the signed referral form that your medical provider gave you on your first visit.

How to send your referral to our team of physical therapists?

  • Once you refer your patient, one of our physical therapists will perform a full exam and create a specific treatment plan for your patient.
  • Since you are the PCP on file, we will forward a copy of this exam and treatment plan for your records.
  • Our typical treatment plan lasts eight weeks, during which your patient will get evidence based treatment based on the diagnosis.
  • At the end of those eight weeks, we will perform a re-evaluation to determine how much your patient has improved.
  • We encourage the patient to schedule a follow up with their primary care physician to update them on their improvement and for any refills of medication.
  • Your referral would be limited to physical medicine, and we will refer the patient back to you at the conclusion of their treatment.
